This method is mostly used in younger patients with early symptoms of aging because the re is no effect on the general features of the face and the neck.

Procedure is mostly used for saggings around the eyes and the cheeks. The cut is made under the hair line around the temple; skin and the soft tissue under the skin is retracted over the bones.
If there is accompanying drop of eyelids or thin lines around the eyes; the lift can be made with an added cut around the temple. Upper eyelid problems can be solved in the sam surgical session.

Yaşla birlikte yanaktaki yağ dokusu azalır, yanaklar aşağı sarkar, yüz ovalliği azalır, alt göz kapağındaki torbalar belirginleşir, yanak ile göz kapağı arasında derin bir hat oluşur. Bu görüntü insanların yorgun ve yaşlı görünmesine neden olur. Ayrıca sarkan yanaklar burun ile dudak kenarları arasında iki derin çizginin oluşmasına neden olur.
Orta yüz germe ameliyatında amaç kaybedilmiş olan yumuşak dokuyu yerine koymak, aşağıya doğru sarkmış olan yapıları doğal anatomik pozisyonuna geri taşımak ve bozulmuş olan alt göz kapağı yanak ilişkisini düzeltmektir. Bu yöntem yüzün genelinde ve boyun bölgesinde etki sağlamadığı için genellikle yaşlanma belirtilerinin yeni başladığı genç yaştaki hastalarda tercih edilmektedir.
Özellikle göz çevresi ve yanak bölgesinde oluşan sarkmalarda tercih edilen bu teknikte şakak bölgesinde saç içerisinden yapılan kesi ile yüze ulaşılmakta ve cilt ve cilt altı dokular yüz kemikleri üzerinden serbestleştirilerek gerilmektedir.
Kaşlarda düşüklük veya göz çevresinde kırışma ve sarkma var ise şakak bölgesinden küçük bir kesi yapılarak kaş kaldırılır, şakak bölgesi de gerilir. Üst göz kapağında torbalanma var ise aynı seansta bu bölgedeki sorunlar da giderilebilir.
